Herpes is an infectious disease, very contagious, usually benign, caused by two kinds of viruses from the family Herpesviridae [Herpes simplex virus 1 (HSV-1) and Herpes simplex virus 2 (HSV-2)]. It is characteristic of these viruses to infect some cells causing lesions, with destruction of the cell, in the epithelium (cells of the skin and mucous membranes) and in neurons (cells of the nervous system) from where they are reactivated by several factors such as exposure to the sun, fever, menstrual period, trauma, stress, drug treatment or situations that reduce physical resistance. Intervals of relapse are varied; it is believed that these viruses remain in our body throughout life.
The transmission of the virus takes place preferably by direct contact, person to person, even if no active lesions. Infection through objects can exist, but is less common. The time between contact and the initial symptoms (incubation period) is estimated at two weeks. Around 90% of people had or will have contact with HSV-1 and about 20% with HSV-2.
In most cases HSV-1 causes lesions around the mouth (cold sores, gum infection, herpes pharyngitis), HSV-2 causes genital lesions (urethra, vulva, penis, vagina, etc.). The primary infections with HSV - 1 is a disease primarily of children, while HSV-2 are transmitted mainly through sexual contact, affecting in the majority of cases adults.
A considerable proportion of people who are transmitting herpes virus have no symptoms in both diseases by HSV - 1 and in HSV-2, even the initial infection can elapse without any complaints. People who have systemic symptoms (symptoms than those located) may complain of malaise, fever and discomfort or vague complaints of low intensity.
In cases of HSV-1 the symptoms can be: gum infection, high fever, multiple wounds in the mouth and tongue, pharynx, severe inflammation of the gums and swollen glands in the neck. In adolescents and older children can even cause pharyngeal ulcers.
In cases of HSV-2 the symptoms can be; wounds in the penis, vulvas, vaginal, anal, with or without urethral involvement, what can cause discomfort when urinating. The diagnosis of genital herpes in children is to think of sexual abuse. Even beyond the neonatal period the reactivation of HSV-1 or 2 can cause neurological damage to important framework similar to that of other viral infections.
In cases of neonatal infection can be acquired either by getting the infection during pregnancy as by contamination during childbirth. The neonatal diseases when generalized are very serious. It is important to consider the need for the birth by caesarean section in all cases of genital herpes.
In typical cases is sufficient clinical examination. In special situations it may be necessary to identify the virus by examination of the lesion or even laboratory research of other affected organs.
